At all, practice sessions, seminars, tournaments and events conducted by Illinois Taekwondo State Organization NFP (ITSO); reasonable care is taken to prevent serious injuries and to minimize accidents. I recognize and acknowledge that there are certain risks of physical injury to participate in any of the taekwondo trainings/events held by ITSO and I agree to assume the full risk of any such injury, damages or loss regardless of severity which I or my minor child/ward may sustain as a result of participating in any activities associated with any such membership. I hereby release, discharge and further agree to indemnify, defend and hold harmless ITSO, it's Board of Directors, officers, agents, members, employees and guests from any and all claims from injuries, damage or loss which I or my child/ward may accrue on account of participation in taekwondo training/events.

Recognizing the strenuous nature of the activities offered at Illinois Taekwondo State Organization NFP (ITSO), I hereby acknowledge that the member(s) listed above are in reasonable good health. I hereby give permission to ITSO to authorize medical treatment in the event of an emergency or injury, in the absence of a legal guardian or parent.

I grant Illinois Taekwondo State Organization NFP (ITSO), its representatives, Board of Directors, Officers and staff to take photographs of me and my property in connection with ITSO events, seminars, tournaments and training sessions. I authorize ITSO, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that ITSO may use such photographs of me with or without my name and for any lawful purpose, including publicity, illustration, advertising, and web content.

ITSO Memberships are non-transferable and non-refundable.
